Referral Form to ADR-LINK

Referral Form for Societies in the South West Region of Ontario

 

Use this form to refer a case for ADR. While the suggestion for ADR can be made by any party including the court, the referral must come from the children's aid society. More information on the referral process can be found on this web site.

Application for ADR-LINK roster

Practitioner Information Form for ADR-LINK

 

ADR-LINK maintains a roster of qualified practitioners available to accept child protection ADR cases in the South West Region of Ontario. If you are an ADR practitioner who wishes to be placed on this roster, the information you provide on this forms helps us match referred cases to your skill set and geographic availability. You will be asked to provide a copy of your current insurance coverage. NB: In most cases, to qualify for the ADR-LINK roster, an ADR practioner must be listed on the roster of either the Ontario Association for Family Mediation or theGeorge Hull Centre. 2 p.

Feedback form for Families

Feedback Form for Non-CAS Participants in FGC

 

This feedback survey is for the families and others participants in any Family Group Conference organized through ADR-LINK. Participants can complete this survey once the conference is finished. ADR-LINK needs the opinions of participants to ensure that the FGC service meets the needs of the families and other participants. ADR-LINK will keep this information confidential and not reveal comments to the CAS or to the FGC Coordinator.

Feedback Form for Non-CAS Participants in Mediation

 

This feedback survey is for the families and others participants in any mediation organized through ADR-LINK. Participants can complete this survey once the mediation concludes (with or without a settlement). ADR-LINK needs the opinions of participants to ensure that the mediation service meets the needs of the families and other participants. ADR-LINK will keep this information confidential and not reveal comments to the CAS or to the Mediator.

Feedback Form for CAS Workers

 

As a CAS worker referring a case for either mediation or family group conferencing, ADR-LINK needs your opinions and feedback about the process and outcome. Please complete this 2-page survey for each referral at the close of the ADR process, even if there was no mediation session or family group conference. This information is collected to aid the Ministry of Children & Youth Services as it plans for the next steps of child protection ADR in Ontario.

As a Coordinator conducting a Family Group Conference for ADR-Link in the South West Region of Ontario, this form asks you to describe case-specific details of the conference process and outcome and provide feedback on the referral process. Complete this form for each referral as you close the file, including those not ending in a conference. This information is collected to aid the Ministry of Children & Youth Services as it plans for the next steps of child protection ADR in Ontario.

As a CP Mediator taking cases for ADR-Link in the South West Region of Ontario, this form asks you to describe case-specific details of the mediation process and outcome and provide feedback on the referral process. Complete this form for each referral as you close the file, including those not ending in a mediation. This information is collected to aid the Ministry of Children & Youth Services as it plans for the next steps of child protection ADR in Ontario.

Non-ADR-LINK Forms

Form 0011, Office of the Children's Lawyer

Form 0011: Notice Where Alternative Dispute Resolution is Proposed Under the Child & Family Services Act

 

Use this "fill and print" form to notify the Office of the Children's Lawyer of the intention to make a referral for court-related ADR. You will be required to include the names and dates of birth of the children, your contact information, names and contact information for parents/caregivers, names and contact information of any other expected participants, nature of court proceeding, description of protection concerns, issues proposed for ADR, and the nature of any on-going criminal investigation and/or prosecution. If the proposed ADR is in relation to an openness order, you will be asked to attach a copy of that order in your submission. 4 p.

 

This form is to be sent by the children's aid society via fax to the Office of the Children's Lawyer, attn: ADR Intake Co-ordinator.
